Some schools ask you to complete a SIF to provide extra information for the admissions criteria to be looked at fully. This is in addition to the Common Application Form being submitted.

Supplemental Information We encourage all university/college transfer applicants to complete and submit a TMU Supplementary Form. All applicants, except current Ontario Secondary School Students (OUAC 101 applicants), are encouraged to complete and submit a Supplementary Form.

If you are applying to certain schools for religious reasons, you will need to submit an appropriate Supplementary Information Form (SIF) to the school by the deadline.

* Outline your previous research training, experience (field work, practical experience) and publication history. * Describe your relevant training and work experience (non-university related) since you entered university, including travel. * Provide any other information you feel may interest the admissions committee.

The SIF will enable the school to determine if your child meets their admissions criteria. Failure to complete the SIF is likely to result in your child not being offered a place at the school. The SIF will need to be completed and returned to the school by the date specified on the form.

Supplementary Information Form Exceptional Social/Medical need. Applicants who wish to be considered for priority under the criterion of exceptional social/medical need at a community or voluntary controlled school must complete this form, in addition to the local authority application form.
