8. In Section 6, articulate your reasons for applying to the chosen programs, while mentioning any relevant details for the review of your application. If applicable, include a statement for readmission or reinstatement.
9. Finally, in Section 7, sign and date the form to confirm that all information provided is accurate and complete.
