How to fill out the Mini Mental State Examination (MMSE) online

The Mini Mental State Examination (MMSE) is a widely used tool for assessing cognitive function. This guide provides a clear and supportive walkthrough on how to complete the MMSE form online, ensuring that users can accurately assess cognitive abilities effectively.

Follow the steps to fill out the MMSE online.

  1. Press the ‘Get Form’ button to obtain the MMSE document and open it in your chosen online editor.
  2. Read through the introductory instructions carefully. Begin by filling in the patient’s name, date, CPR number, and the name of the evaluator. Make sure all required information is accurately provided.
  3. Move to the orientation section. Answer the questions regarding the current date, season, and day of the week. Each correct answer will receive one point.
  4. Proceed to the language, praxis, and construction section. Here, name a pencil and a clock. Then, repeat the phrase 'No over, under, or beside.' Make sure to follow this step accurately, as both parts are crucial.
  5. In the registration section, ask the patient to name three objects, allowing one second for each answer. Scores will be totaled based on correct responses.
  6. Follow the three-step command: 'Take the paper in your right hand, fold it in half, and place it on the floor.' Each completed step adds to the scoring. Also, obey the instruction to 'Close your eyes' and ensure it is done.
  7. For the attention and calculation part, conduct the '100-7' subtraction test, stopping after five responses or alternatively spelling the word 'MONEY' backward. Points are awarded for each correct response.
  8. Ask the patient to write a complete sentence and then replicate a provided drawing. Scoring is again important, so take care with evaluations.
  9. After filling out all the necessary sections, ensure to tally the total score, which has a maximum of 30 points. Review all entries for accuracy.
  10. Finally, you can save the changes made, download the document, print it, or share it as needed.
