10. List your employment history for at least the last seven years. Be sure to include reference contact details if required and indicate if you allow your current employer to be contacted.
11. In the education section, list all relevant academic qualifications, diplomas, and certifications. Ensure to provide copies of your highest degree and transcripts as required.
12. Complete the declaration section by certifying that all information provided is accurate. Sign and date the form to confirm your submission.
13. In the authority to release personal information section, authorize the release of your information and ensure you meet the requirements for witnesses or notary seals as indicated.
