Follow the steps to fill out your reimbursement form accurately.

1. Click ‘Get Form’ button to obtain the Contactless Payment Reimbursement Form and open it for completion.
2. Fill out sections 1 to 4 of the form in capital letters using a black pen, or type your details into sections 1-3, print the form, and then sign section 4.
3. In section 1, enter your customer details including your title, family name, given name, email address, and contact number. Make sure to include the country and area code for the contact number.
4. Proceed to section 2 to enter trip details. Indicate the trip date, time of the trip, tap-on location, destination, fare charged, estimated reimbursement amount, and select the reason for reimbursement.
5. If your payment was made using a payment card, complete section 3.1 with the payment card number (first six and last four digits), name on the card, and expiry date.
6. For mobile payment devices, also complete section 3.2 by providing the device account number and last four digits.
7. When you reach section 4, read the reimbursement agreement carefully. After reading, sign and print your name along with the date.
8. Once all sections are completed, review the information for accuracy, then save your changes, download, print, or share the form as needed. Finally, return the completed form to Opal Customer Care.
