9. List the gratuity nominee details, including their name, date of birth, relation, sex, percentage of gratuity, bank details, mobile number, email ID, and address.
10. Specify the total service period, non-qualifying service details, net qualifying service period, and qualifying six-monthly details.
11. Lastly, fill the LTA nominee details and family details sections as required, ensuring to include the names, relations, and any relevant information regarding disabilities.
