- Complete the Pressure Buffer section calculating A-B, which should be greater than 3.0 psid.
- Revisit the requirements for the Double Check Valve Device, confirming closures on Check Valve #1 and Check Valve #2.
- Enter the details of the Pressure VB / Spill Proof VB assessments as done in prior sections.
- Fill in the Remarks section to note any additional observations or comments regarding the test.
- Complete the Testing Company information, including Phone Number, Cell Number, Company Address, E-mail, and Fax Number.
- Provide the relevant licenses and their expiration dates, including VA Plumbing License # and Backflow License #.
