8. Document the date infused or note if the patient was not infused. Input the date of any scheduled future infusion and sign the checklist to confirm the completion of the review process.
9. Submit the Pre Infusion Checklist online or fax it to Biogen Idec, ensuring you retain a copy in the patient's records for reference.
