Contact support

The ASQ-3 questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills. The ASQ screening can help identify your child's strengths and any areas where your child may need support. As a parent or caregiver, you know your child best.

The Ages and Stages questionnaire, or ASQ, is a reliable, researched based screening tool for you to review your child's development. These questionnaires will be used at key points by your health visiting team as your child grows. If you need support in completing this, please speak to your health visiting team.

In the ASQ-3, higher scores indicate more positive outcomes. Each version of the ASQ-3 has different cutoff scores that indicate whether the child's development appears to be on schedule. On the summary sheet of the version you administered, use the bubbles in Item 1 to fill in the total score for each area.

Early childhood education programs, pediatric offices, physical and occupational therapists, speech-language pathologists, state and local organizations, and children's families all use the ASQ® assessment to screen children for developmental delays and help them get the early intervention services they may need.

Developmental and social-emotional screening in the childcare setting is most beneficial when ASQ questionnaires are completed independently by both the teacher and the parent. This way, skills and behaviors can be observed in multiple settings, and then compared and discussed in a collaborative manner.

Developmental Screening The American Academy of Pediatrics (AAP) recommends developmental and behavioral screening for all children during regular well-child visits at these ages: 9 months. 18 months. 30 months.

To print “What is ASQ-3?” or “What is ASQ:SE-2?” from ASQ Online: 1. From the main menu select “Screening Management.” 2. From the left menu under “Documents” select “View/Print Documents.”

On ASQ:SE-2, the Overall questions cover general concerns related to eating and sleeping behaviors, such as, “Do you have concerns about your child's eating or sleeping behaviors?” A question about concerns related to toilet training or toileting behaviors is included on the 30, 36, 48, and 60 month questionnaires.

Each of the 5 developmental areas of the ASQ-3 is scored separately. The total for each is compared to a unique cutoff score that determines whether a child is low or no risk, in the monitoring zone, or below the cutoff and should be referred for further evaluation.
