- Complete the 'Social Issues' section by circling the number that reflects how often you experienced specific feelings related to your foot problems.
- Finally, in the 'Subject Comments' section, provide any additional feedback regarding the form’s clarity, difficulty, or any issues related to your foot problems that may not have been addressed.
