This guide provides clear instructions on filling out the Ritm online form for participants in the National External Quality Assessment Scheme. By following these steps, users can ensure accurate and complete submissions to the health department.

Follow the steps to successfully complete the Ritm online form.

1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Begin by selecting whether you are a new or old participant. If you are an old participant, be sure to indicate your laboratory code in the provided space.
3. In section A, enter your laboratory information in bold letters without using abbreviations, including the name, address, zip code, region, contact person, their position, email (which is mandatory), and telephone numbers.
4. Check all applicable types of testing site your laboratory operates under in section A.
5. In section B, list the names of relevant personnel, including the pathologist and chief medical technologist or quality assurance officer. Include the name of the HIV proficient medical technologist and their proficiency certification number.
6. Indicate the assigned section for the laboratory in section B, selecting from diagnostic, blood bank service, or both.
7. In section C, check the serologic tests your laboratory would like to participate in and specify the methods used (Rapid or EIA) for each test.
8. Fill out section D with the required annual census information, including total tests done, reactive screening tests, and positive confirmatory tests for each type of test.
9. In section E, address the availability of a courier service near your area. If none is available, provide an alternative address where test panels can be sent.
10. Complete section F by noting your laboratory's performance rating from the previous year for each test type.
11. Finally, agree to abide by the rules of participation by signing and providing the name and position of the head of agency or relevant veterinarian.
12. After reviewing all entered information, save your changes, then download, print, or share the form as necessary.
