10. When filling in children’s information, provide their date of birth, name, gender, birth certificate or notification number, and preferred medical facility.
11. Attach copies of identification cards for both the contributor and spouse. For children, include copies of their birth certificates or notifications.
12. Add a recent passport-sized photo for each person named. Write their name and ID number on the back of each photo.
13. Complete the declaration section by signing with the contributor's name and date. An authorized official must also sign in the respective area.
14. Finally, save your changes, and download, print, or share the completed form as needed.
