- Move to the beneficiary section. Revoke your previous beneficiary nomination and list the primary beneficiary's name exactly as it appears on their Social Security card, along with their Social Security number and mailing address.
- Detail your relationship to the primary beneficiary, including their birth date and marriage date if applicable.
- For contingent beneficiaries, write down their name, Social Security number, mailing address, and details about your relationship to them as well.
- If you are requesting a name change, state your new full name as it should appear on your Social Security card. Select the reason for the change from the provided options.
- Collect and send necessary documents such as a copy of your Driver’s License or Birth Certificate, Social Security Card, marriage license, divorce decree, or adoption documents as applicable.
- Complete the signature section by signing as the member and including the date. Additionally, have a witness sign the form.
- Send the completed original form to the LOPFI address provided. Note that faxes are not accepted.
