7. Provide your contact information, including surname, title (if applicable), forenames or trading name, address, postcode, country, and optionally a fax/phone number and email.
8. Indicate whether you authorize the disclosure of parental cultivar epithets by checking the corresponding box.
9. Confirm if you are enclosing a colour photograph as required for primary hybrids, by selecting yes or no.
10. Complete the declaration regarding the originator. Choose one of the three given options based on your status in relation to the orchid hybrid, and provide further required details.
