14. Attach any required documents including the St. Louis Regional Office Budget Sheet and the UR recommendation sheet, ensuring they are linked to your form.
15. Request approval or denial by having the necessary supervisor signature and provide the formal approval status.
16. Complete the total request and total cost areas by adding any costs related to the services requested.
17. Once all fields are completed and verified, users can choose to save changes, download, print, or share the form as needed.
