7. Complete the evaluator's information, including their name, title, hospital or clinical facility, mailing address, city/state, zip code, phone number, and date. Make sure to include the evaluator's signature and the date.
8. Ensure that the completed form is returned in an official sealed envelope with a signature across the envelope closure to the applicant for inclusion in their supplemental application packet.
