  8. Calculate Total B by adding the closing balance and ineligible use, then subtract Total B from Total A to determine Total C, which is the total fuel claimed for refund.
  9. Proceed to Part B, detailing how Total C fuel was used, providing information for all vehicles, unregistered vehicles, commercial purposes, and commercial vessels.
  10. If additional space is needed, continue on page 2 or attach a separate schedule with the required information.
  11. Upon completion, authorize the application with your signature, name, and phone number. Ensure you date the application and include any additional necessary documentation.
