4. Proceed to Policy #2 for the organizational chart. Ensure that you document the program's structure, highlighting lines of authority and responsibilities, completing blank fields as necessary.
5. In Policy #3, outline the instructor monitoring requirements. Provide the individual who will be responsible for this monitoring and any verification processes that will be used.
6. Move to Policy #4 and set the student to instructor ratio, clearly stating adherence to no more than 15 students per instructor during clinical training.
7. In Policy #5, document the procedures for handling student absenteeism and makeup classes. Clearly specify who will be approving absences and how makeup assignments will be managed.
8. Fill out the attestations A-H at the end of the form. These sections confirm understanding of regulatory requirements and must be signed by the appropriate individuals, including the Administrator and Director of Nursing or Program Director.
