8. In Section III, indicate the fuel types for which you will file quarterly returns by checking the corresponding boxes. Also specify whether you withdraw fuel from bulk and if you haul petroleum products.
9. Mail the completed application along with the required documentation to the address provided. Include a $10.00 filing fee with the application, ensuring it is made payable to the Motor Vehicle Division and includes your MVD Account Number.
10. After completing all sections and ensuring accuracy, sign where indicated. Collect any additional signatures required from partners or officers.
