13. Include the classification code(s) for the listed goods or services, ensuring you check applicable class numbers.
14. If the mark has been previously filed with the U.S. Patent and Trademark Office, provide the relevant application details.
15. Indicate how the mark is used in commerce by checking the boxes that apply.
16. Prepare and attach three identical original specimens demonstrating the mark in use.
17. Complete the Declaration of Accuracy section by signing and dating the application.
18. Once completed, save the changes and choose to download, print, or share the application as needed.
