6. The Notary Public must complete the acknowledgment section of the form. You may need to check with a notary about remote notarization options to meet the requirements if necessary.
7. After completing all sections, scan the signed and notarized form. Email it to grantsreform@its.ny.gov, including an organization chart showing the Head of your Organization and, if applicable, a completed Substitute W-9 form for new vendors.
