The PAQ includes a 20‐item questionnaire that assesses PAD‐related health status along six domains (PAD symptoms, recent changes in PAD symptoms, physical limitations due to PAD symptoms, PAD treatment satisfaction, social functioning, and quality of life).

If you have an ABI of less than 0.90 while resting, you may have PAD. An ABI of less than 0.40 is a sign of severe PAD. If there are problems with the arteries in your ankle, your provider may do a toe-brachial index (TBI) test instead, which measures the blood pressure in your big toe. A TBI less than 0.7 is abnormal.

PAD also increases your risk of stroke, heart attack and other cardiovascular problems. A test result of 1.0 to 1.4 is normal.

Several imaging tests can be used to diagnose PAD: Vascular ultrasound. This exam uses sound waves to create pictures of the arteries and locate blockages. Doppler ultrasound: Doppler ultrasound is a special ultrasound technique that can help detect areas of restricted blood flow through an artery.

The ankle-brachial index (ABI) test is usually the first test used to diagnose PAD. The test compares blood pressure in your ankle with the blood pressure in your arm. Your provider uses a blood pressure cuff and ultrasound device for this painless test.

In healthy individuals, the lower limbs should be symmetrically warm, suggesting adequate perfusion. A cool and pale limb is indicative of poor arterial perfusion.

Physical examination findings suggestive of PAD include abnormal pulses, audible bruits, nonhealing lower extremity wounds, lower extremity gangrene, elevation pallor, dependent rubor, delayed capillary refill, and cool extremities ( Table 2 ). Patients with one or more of these findings should undergo ABI testing.

Traditionally, ABI values of 1.00 to 1.3 are considered normal. ABI values of 0.00 to 0.40 indicate severe PAD and 0.41 to 0.90 indicate mild to moderate PAD, values of 0.91 to 0.99 are considered borderline, and values greater than 1.30 indicates noncompressible arteries.
