Follow the steps to successfully complete your registration form.

1. Click ‘Get Form’ button to obtain the form and access it for editing.
2. Select the type of account you wish to open by checking the appropriate box. Options include Individual, Joint, UGMA/UTMA, Guardian, Organization or other entity, and Trust. Ensure that you check only one option and verify its validity in your state.
3. For organization accounts, complete section 2 by providing organization information. This includes checking the registration type, providing the organization's name as shown on federal tax documents, and the employer ID number.
4. In section 3, fill out the account owner information. Provide the full legal names, birth dates, email addresses, Social Security numbers or individual taxpayer ID numbers, citizenship status, and mailing addresses for each account owner.
5. If applicable, complete section 4 for trust accounts. Provide the trust's name, employer ID or Social Security number, date of trust agreement, and type of trust.
6. In section 5, specify the Vanguard funds you would like to invest in by entering the fund names, symbols, and the amounts you wish to invest.
7. Indicate how you would like to make your initial investment in section 6. Options include by check, electronic transfer, or wire. Ensure to complete any necessary sections if you select electronic transfer.
8. Optional section 7 allows you to choose your distribution options for dividends and capital gains. Make sure to select one option that fits your preferences.
9. If you desire to set up an automatic investment plan, fill out section 8 with your chosen schedule and the Vanguard funds involved.
10. Complete section 9 with your bank information if you selected electronic transfer or automatic investment. Ensure to include the bank name, account type, and required documentation.
11. If desired, fill out section 10 to request checkwriting privileges, providing necessary fund information and signatures.
12. Finally, review section 11 where all account owners must authorize the form. Ensure you sign and date as indicated.
13. Return pages 1–11 of the completed form in the enclosed postage-paid envelope or to the designated address provided for Vanguard.
