- In the EFT Banking Information section, input your Bank Account number and ABA Routing number.
- Enter the Account Name and select the Account Type as Checking or Savings.
- Fill in the Bank Name and address (street, city, state, zip).
- Ensure to submit a copy of a voided blank check or a bank letter as page 2 of this EFT request.
- By signing below, authorize Caremark to make electronic credit entries to the designated account. Make sure that the person authorized to sign is either the Pharmacy owner, a Corporate Officer, or an Authorized agent.
- Print the completed form, sign it, and fax it to Caremark at the provided fax number: 480-614-7443.
- After submission, please allow 30 days for the establishment or change of your Caremark EFT set-up.
