Initial CPAP settings: 5-8cmH2O, however in some clinical conditions (e.g. bronchiolitis, severe chronic lung disease and tracheal issues) higher CPAP up to 12-14cmH2O may be ordered by the NICU consultant.

What Do CPAP Pressure Settings Mean? The key is to ensure that your CPAP device settings are appropriate for your specific condition. For most people, an appropriate CPAP pressure is between 6 and 14 cmH2O, with an average of 10 cmH2O.

There are different methods of delivering CPAP therapy, such as bubble CPAP, variable flow CPAP, and ventilator-derived CPAP. Bubble CPAP is a simple and economical mode of delivering CPAP in neonates, including those born preterm and those with a low birth weight.

During the first 30 seconds: provide warmth by placing infant under a radiant heat source, position the head in the “sniffing” position, clear the airway as necessary (bulb syringe, suction catheter), dry the infant, and stimulate breathing.

The current (8th) edition of the American Academy of Pediatrics—Neonatal Resuscitation Program (NRP) textbook recommends administration of a volume expander if the neonate is not responding to initial steps of resuscitation and there are signs of shock or a history of acute blood loss [1].

Pending additional studies to identify the ideal flush volume in newly born humans, NRP 8th edition recommends a 3-mL flush volume for babies of all gestational ages after intravascular epinephrine administration. Administer the epinephrine rapidly, followed by rapid administration of the 3-mL normal saline flush.

CPAP delivers air to a baby's lungs either through small tubes in the baby's nose or through a tube that has been inserted into the windpipe. The tubes are attached to a machine, which helps the baby breathe. Babies on CPAP breathe on their own, the machine does not breathe for them.

A gas (oxygen) flow rate of 5–10 L/min is required for older children with pneumonia, while 3–4 L/min may be sufficient to generate CPAP in small neonates. In premature neonates < 32 weeks' gestation, pure oxygen is not safe as a high concentration can cause retinopathy of prematurity.
