The father's name can be added to his child's record (if no other man's name appears on the record) by the mother and father completing a form known as an "Affidavit Acknowledging Paternity". This form may be requested by writing to the address above. This is a legal document.

To request a paper copy of a certificate of live birth or for further details about correctly documenting an out of facility birth/home birth, contact the Bureau of Vital Records at 573-751-6387, Option 3.

Contact BVR at (573) 751-6387 or visit https://health.mo.gov/vitalrecords/ if you need a rescission form. When the rescission is filed, the man will no longer be the legal father; however, his name will stay on the birth certificate unless a court order tells BVR to remove his name.

To apply for an amended birth certificate the applicant should submit the certified copy of the court order, a written request for the birth certificate (stating where the new certificate is to be sent), and the applicable fees to Missouri Dept. of Health and Senior Services, P.O. Box 570, Jefferson City, MO 65102.

Call the Missouri Bureau of Vital Records at 573-751-6378 and state that you want to add the father's name to the birth certificate. You must provide your name and legal mailing address to receive the required form, “Affidavit For Acknowledging Paternity.”

I do solemnly declare and affirm that I am the natural father of the child listed on this Affidavit and that the statements are true under the pains and penalties of perjury. I consent to this Affidavit and request that my name and other information be added to this childʼs birth record. THIS IS A LEGAL DOCUMENT.

You can open a parentage case with the Court. Once the legal parents are established by the Court, follow the procedure on the State Dept. of Public Health website to request that the birth certificate be amended to include the father's name. You and the other parent can sign a Voluntary Declaration of Parentage .
