This guide will walk you through each step of the form, ensuring you provide accurate and comprehensive information.

  1. Press the ‘Get Form’ button to access the document and open it in your online editor.
  2. Begin by entering the reader’s name in the designated field followed by their grade level. Make sure to input accurate information as this is crucial for the assessment.
  3. Record the date of the assessment in the provided space. This helps in tracking the progress over time.
  4. Next, input the accuracy rate based on the reader's performance. This is an important metric for understanding their reading capability.
  5. When conducting the oral reading, you will instruct the reader using the Book Introduction section. Ensure that you point out where the reader should start and how to approach the reading segment.
  6. As the student reads, take notes on their miscues in the Running Record section for the first 100 words. Document any errors alongside the text for thorough analysis later.

  8. Encourage the student to retell or summarize the text they just read. Make notes on this section as it is important for assessing their comprehension.
  9. Proceed to the Comprehension Questions Section where you will evaluate the student’s responses to specific queries regarding the text. Record their answers accurately.

Steps to Analyzing a Running Record Step 1: Is this text at an appropriate level? ... Step 2: What cue sources did the child use? ... Step 3: Did the child integrate the cue systems? ... Step 4: Is the child monitoring? ... Step 5: What other strategic activity did the child use at difficulty/after error?

Usually, your child's teacher will determine their Lexile reading level and then choose books that have a matching score. The Lexile score, or measure, describes a child's reading ability and matches them with books and other reading materials. This measure ranges anywhere from 0L to 2000L.

INDEPENDENT LEVEL: text in which no more than approximately 1 in 20 words is difficult for the reader. Independent-level texts are appropriate if students are reading independently with little or no instructional support. Independent-level texts are often used to build fluency.

Place a check mark above each word that is read correctly. If the child reads incorrectly, record above the word what the child reads. If the child is reading too fast for you to record the running record, ask him or her to pause until you catch up. Be sure to pay attention to the reader's behavior as he or she reads.

The observer writes down everything possible that the child says and does during a specified length of time or during a designated activity. Record all observed behavior, in great detail: actions, body and verbal language, facial expressions, etc. Remember, this is only what is directly observed!

To complete a running record, a child will read aloud a text that the teacher estimates to be at their independent level. While the child reads, the teacher takes notes on the accuracy of the reading and marks down any deviations from the original text using a standard coding system.

Sometimes you'll find the reading level of a book displayed on the book jacket. You can also easily find the reading level of a book at The Scholastic Store: Simply refer to the “Product Details” section of any product page to find relevant information for several leveling systems.

One example of a running record would be if the text reads "The cow jumped over the moon," and the student read "The car jumped over the fence," then the student made two errors out of six words. The teacher would record a checkmark, the word car, two more checkmarks, and the word fence on the running record form.

Guided Reading Levels (GRL) GRL is a guided reading system used in some schools. To determine reading levels using GRL, children sit one-on-one with their teacher and read from a book that's considered standard for their grade level—a “benchmark” book. GRL books range from A to Z with A being the easiest.

percentages (i.e., 95% or higher = independent level; 90%–94% = instructional; below 90% = frustrational); in the above example, the student would be at an instructional reading level (94% falls in the 90% to 94% range).
