2. Begin with Section 1, which requires Student Details. Fill in the student's family name, given names, preferred name, date of birth, gender, citizenship, country of birth, and first language. Attach a copy of the student's birth certificate or passport as required.
3. Continue to Section 2 to provide details about the father or guardian. Include name, relationship to the child, postal and residential addresses, contact details, and employment information.
4. Next, move to Section 3 for the mother or guardian's details, repeating the same type of information required in Section 2.
5. Proceed to Section 4, which focuses on the Student Needs Profile. Answer questions regarding any academic support your child has received, medical conditions such as allergies, and regular medications your child may take.
6. In Section 5, complete the Family Circumstances section if applicable, providing information on custody and parental relationships.
7. Section 6 requires you to fill in details about siblings and relatives who have attended Mueller College. Include their names, relationships, and years of attendance.
8. Complete Section 7 by providing contact information for two referees who are familiar with the child and parents, such as a doctor or previous teacher.
9. Finally, in Section 8, both parents or guardians must sign and date the application. Ensure that the Enrolment Application Processing Fee of $30 is included and all required documents are ready for submission.
10. After completing the application, you can save your changes and download or print the form for submission. Ensure it is mailed to the college address provided.
