Change of Address Notification Required. All foreign nationals, including legal permanent residents (green card holders) who are not U.S. citizens are required by Section 265 of the Immigration and Nationality Act to notify the USCIS of address changes while in the United States.

As the person completing Form I-864, you are the sponsor (or, as appropriate, a substitute sponsor or joint sponsor). By signing this form, you agree to financially support an immigrant who applies for either an immigrant visa or adjustment of status to become a lawful permanent resident.

2. Who Completes Form I-864EZ? Only the U.S. citizen, lawful permanent resident, or U.S. national who filed Form I-130 for a family member, may complete this affidavit. A sponsor is required to be at least 18 years of age and domiciled in the United States, its territories, or possessions.

The purpose of Form I-865, Sponsor's Notice of Change of Address, is to report a sponsor's new address. If you, as a sponsor, change your address, you must submit a completed and signed Form I-865 to U.S. Citizenship and Immigration Services (USCIS).

In order to change the address on your permanent resident card, you must contact Citizenship and Immigration Canada. You can call them at 1-888-242-2100, or you may notify them of a change of address via snail mail. You may also be eligible to change your address using the online service.

For U.S. citizens or U.S. nationals, a copy of your birth certificate, passport, or certificate of naturalization or citizenship. For lawful permanent residents, a copy of both sides of your Form I-551, Permanent Resident Card.

If you are a U.S. citizen or a lawful permanent resident and are sponsoring a family member to immigrate to the United States, you will likely need to file one or more of the I-864 Affidavit of Support forms with the Department of Homeland Security (DHS).

Income Requirements You must show that your household income is equal to or higher than 125% of the U.S. poverty level for your household size. (Your household size includes you, your dependents, any relatives living with you, and the immigrants you are sponsoring.)
