8. Specify the outage duration by entering the start date and time, as well as the end date and time.
9. In the 'Describe Outage Purpose/Building(s) Affected' section, explain the purpose of the outage and list all buildings that will be affected.
10. Provide information for the PPD use section regarding recommended trades and approval signatures, including affected buildings and areas.
11. Confirm if additional sheets are attached and indicate this in the checkbox provided.
12. Enter your name and contact number in the 'Outage Supervisor' section and provide your signature.
13. Once all fields are completed, save your changes, download, print, or share the completed form as required.
