Definition. Physical examination is the process of evaluating objective anatomic findings through the use of observation, palpation, percussion, and auscultation. The information obtained must be thoughtfully integrated with the patient's history and pathophysiology.

The H&P: History and Physical is the most formal and complete assessment of the patient and the problem. H&P is shorthand for the formal document that physicians produce through the interview with the patient, the physical exam, and the summary of the testing either obtained or pending.

The H&P, or the “History and Physical,” is a term used to describe a physician's examination of a patient. In an H&P, the physician obtains a thorough medical history from the patient, performs a physical examination, and then documents their findings.

The HPI should be written in prose with full sentences and be a narrative that builds an argument for the reason the patient was admitted. Has a starting point (i.e. “the patient was in her usual state of health until 5 days prior to admission.). Has appropriate flow, continuity, sequence, and chronologic order.

The History and Physical Exam, often called the "H&P" is the starting point of the patient's "story" as to why they sought medical attention or are now receiving medical attention.

The H&P consists of two parts. The first is a thorough medical history prompted by questions from the practitioner (and any prior medical records that may have been provided). The second portion is the physical exam, which allows the practitioner to assess the patient's current health and address the chief complaint.
