DD Form 214, Discharge Papers and Separation Documents Information shown on the Report of Separation may include the service member's: Date and place of entry into active duty. Home address at time of entry. Date and place of release from active duty. Home address after separation.

All new users will need to file a DD Form 2875, a User Agreement, a copy of the user's DoD Cyber Awareness Challenge certificate, and the DPAS Security roles form to receive access to the system. If a DPAS account has been deleted due to inactivity, all three forms must be completed as new and resubmitted.

(19b) Signature of Information Systems Security Officer (ISSO) or. Appointee. Signature of the ISSO or Appointee of the office responsible for approving access to the system being requested. (19c) Date. The date the ISSO or Appointee signs the DD Form 2875.
