6. Decide on your shipping option; you may provide a DHL or FedEx account number for shipment or check the box for DHL shipping for an additional charge of $25.
7. Mail your completed application to the address provided or alternatively, send it via email in PDF format along with the photograph in JPG format. Label all files appropriately with the applicant's name.
8. Review your application to confirm all information is complete and accurate before sending.
