- In the fields designated for ‘Paid Hours Not Worked,’ enter the appropriate values (4 or 8 hours) for types of leave taken, such as Paid Vacation or Paid Bereavement Leave.
- Complete the totals for each week, ensuring that total hours worked are accurately calculated at the bottom of the respective sections.
- Sign and date the bottom of the form to verify that the hours reported are accurate and complete.
- Ensure the supervisor or manager also signs and dates the form, confirming the correctness of the reported hours.
- To submit the form, save changes, then download or print the Biweekly Time Sheet. Ensure the document is printed in portrait format, not landscape.
- Scan the completed time sheet and send it to Human Resources via email by 8:30 A.M. on the Monday following the pay period.
