4. In the Transferor/Old Information section, fill out the details of the current owner. This includes their name, mailing address, FID or license number, city, state, and zip code. The current owner must also sign and date this section.
5. Next, in the Transferee/New Information section, enter the information of the individual or entity receiving the plate. This should include the owner’s name, mailing address, FID or license number, city, state, and zip code.
6. Select the Reassignment option to finalize your transaction. Remember, there is no need to add the plate to the EVR inventory; simply proceed with the transaction.
7. Make a copy of this form for your records and file it in the customer folder. The reassignment transaction will automatically cancel the old plate.
8. Lastly, attach the original reassignment form to the original Registration and Title Application and submit it to the RMV.
