- Enter your Contact Number in the phone/mobile number format.
- Fill in your Mother's Maiden Name and Father's Name for record purposes.
- Select your Gender by marking the corresponding box.

- In Part II, select the Purpose of TIN Application by marking one of the options (A, B, or C).
- In Part III, fill out the Withholding Agent/Accredited Tax Agent Information if applicable, including their TIN and RDO Code.
