  2. Begin by entering the total number of full-time and part-time employees in Line 1. This number should reflect those who performed domestic services or received pay during the payroll period that includes the 12th of each month.
  3. In Line 2, input the total gross wages paid for each quarter. This figure should include salaries, commissions, bonuses, and any remuneration in cash or other forms.
  4. For Line 3, indicate the amount of excess wages exceeding $8,000 paid to each employee during the year.
  5. In Line 4, calculate the total taxable wages for each quarter by adding all entries from Line 13B across all pages.
  6. For Line 5, sum the total taxable wages for the entire year from Line 4.
  7. In Line 6, calculate the tax due by multiplying the total taxable wages from Line 5 by the applicable tax rate.
  8. If your report is submitted after the due date, enter the penalty amount in Line 7. This is $25 per month or any fraction of a month that the report is late.
  9. For Line 8, if tax due from Line 6 is not paid by January 31, compute the interest as 1% per month from the due date until payment is made.
  10. Combine the amounts from Lines 6, 7, and 8 to determine the total amount due in Line 9.
  11. List each employee's social security number and name in Lines 10 and 11. Ensure accuracy as the tax will be affected if social security numbers are not provided.
