How to fill out the Race Stroke Scale online

The Race Stroke Scale is a vital tool used by healthcare professionals to assess the severity of a stroke. This guide provides clear, step-by-step instructions for filling out the Race Stroke Scale online, ensuring accurate and efficient evaluation.

Follow the steps to successfully complete the Race Stroke Scale.

  1. Press the ‘Get Form’ button to access the Race Stroke Scale document and open it in the editor.
  2. Begin by entering the patient's name at the designated field labeled 'Patient Name.' Ensure the name is spelled correctly for accurate record-keeping.
  3. Next, fill in the 'DOB' (Date of Birth) field with the patient's birthdate. This information helps establish the patient's identity and any age-related considerations.
  4. Enter the 'Date' of the assessment to document when the evaluation took place.
  5. Proceed to assess 'Facial Palsy.' Ask the patient to smile and record the outcome based on their ability to show their teeth.
  6. Next, assess 'Arm Motor Function' by asking the patient to extend their arm at a 90° angle while sitting or a 45° angle while lying down. Record the result according to their ability to hold the position.
  7. For 'Leg Motor Function,' request that the patient extend their leg at a 30° angle if they are supine. Document the result following the same criteria as for the arm.
  8. Observe 'Head & Gaze Deviation' by looking for any deviation of the eyes and head to one side. Record whether this is absent or present.
  9. Assess 'Aphasia' by asking the patient to follow two commands: close their eyes and make a fist. Note their performance as normal, moderate, or severe.
  10. Evaluate 'Agnosia' by showing the affected arm and asking the patient questions about it. Document their responses to determine the level of recognition.
  11. Total the scores gathered from each section to arrive at the overall RACE Scale total. Remember, a total above 4 indicates the likelihood of a stroke alert.
  12. Once all fields are completed and reviewed for accuracy, choose the option to save changes, download, print, or share the form as needed.

The RACE Stroke Severity Scale is one of several nationally recognized stroke severity scale options. Like all. stroke severity scales, the purpose of the RACE Scale is to help EMS identify patients whose symptoms may be. due to a Large Vessel Occlusion (LVO), and may benefit from mechanical thrombectomy.

Stroke severity may be stratified on the basis of NIHSS scores as follows (Brott et al, 1989): Very Severe: >25. Severe: 15 – 24. Mild to Moderately Severe: 5 – 14.

Objective. The National Institutes of Health Stroke Scale (NIHSS) is a systematic, quantitative assessment tool to measure stroke-related neurological deficit.

The scale is made up of 11 different elements that evaluate specific ability. The score for each ability is a number between 0 and 4, 0 being normal functioning and 4 being completely impaired. The patient's NIHSS score is calculated by adding the number for each element of the scale; 42 is the highest score possible.
