- Provide details about the storage, treatment, transportation, processing, and disposal facilities. Include specifics on on-site storage, such as size, capacity, and provisions for cold storage.
- List the type of treatment equipment used and the disposal facilities available at your facility. Enter the quantity of recyclable wastes sold to authorized recyclers annually and the number of vehicles used for waste collection and transportation.
- Report any details about accidents that occurred during the year, including the number of accidents, affected personnel, and any remedial actions taken.
- Attach details if necessary.
- Answer questions related to compliance standards for air pollution from incinerators, liquid waste generation, and disinfection methods.
- Review your entries for accuracy. Once complete, save your changes. You may also choose to download, print, or share the completed form.
