The screening code (92558) is an automated pass/fail test, which may be performed by support personnel. CPT code 92587 requires 3–6 distinct frequencies, interpretation, and a statement of the presence or absence of hearing loss and the frequencies affected.

Document each frequency screened in a manner that indicates the decibel it was screened at and whether the result was a PASS or REFER. Use consistent notations with a key indicating which symbols or words denote PASS and REFER so that results are clear to caregivers/guardians and providers.

If your newborn fails the hearing screening, arrange for follow-up testing as soon as possible. If your baby has hearing loss, having them tested and treated right away will likely produce better outcomes for your baby. Your baby will receive ABR and OAE testing at their follow-up evaluation at CHOP.

About 1 or 2 in every 100 babies will not pass the initial hearing screening at birth and will need tests with an audiologist who has experience working with babies. This testing should include a more thorough hearing and medical evaluation.

High risk factors for sensorineural hearing loss in the neonatal period included: intraventricular/periventricular hemorrhage, apnea, family history, major malformations of the head and neck, and possibly hyperbilirubinemia and congenital infection.

A "PASS" result means that the newborn exhibited normal hearing function in both ears at the time of the screen. Both ears must pass a single screening to be considered as an overall passing result. Combining passing results in opposite ears on successive screens does not constitute a passing result.
