Filling out the Document1 online is a straightforward process that allows you to register or renew your participation in the CMC EQAS program. This guide provides clear instructions on each section of the form to assist you effectively.

Follow the steps to complete the registration/renewal form online.

1. Click ‘Get Form’ button to access the registration/renewal form and open it in the designated form area.
2. Fill in the lab number if you are renewing your registration and participated in the previous cycle. If this is your first registration, select the option to indicate that you have never participated before.
3. If you previously participated and wish to rejoin, please provide your old lab number in the specified field.
4. Select the schemes you wish to participate in by ticking the box next to each option. Choose from available schemes such as Chemistry I, Thyroid Hormones & Cortisol, and more. Ensure to review the fee associated with each option.
5. Input the details of your payment method. If you made an online money transfer, provide the reference number, transaction date, and bank name and amount. Alternatively, if you are enclosing a demand draft, include its number and date as well, ensuring it is made in favor of 'The Treasurer, Christian Medical College, Vellore-4'.
6. Complete the personal information section by filling in your name, address, pin code, email ID, mobile number, and signature. Ensure all entries are clear and legible.
7. Once you have completed filling out the form, review all entries for accuracy. You can then save any changes, download, print, or share the completed form as needed.
