5. In the clinical examination area, check each item including ears, eyes, lungs, heart, and abdomen. If there are any abnormalities, note them and indicate if a referral to an AME (Aviation Medical Examiner) is necessary.
6. Assess your visual acuity for distance and near vision. If correction is required to reach the standards, make sure to add the relevant limitation.
7. Provide details regarding your hearing and complete a urinalysis for glucose and protein. Check the applicable boxes.
8. In the 'GP Assessment and Declaration' section, indicate if you meet the medical standards and whether a limitation applies. If necessary, provide comments.
9. Finally, the GP must complete their declaration. Include the GP's name, signature, address, email, GMC number, date, and telephone number.
10. Once you have filled out the form, ensure all information is complete before saving changes, downloading a copy for your records, printing it, or sharing it as required.
