Follow the steps to successfully complete the nomination form.

1. Press the ‘Get Form’ button to access the nomination form and open it in your preferred editing software.
2. In Section A, you will need to provide the recipient's personal details. Enter their surname, given names, gender, birth date, home address, city, postal code, rank, and if known, your MLA representative. Indicate the disciplines applicable to the nominee's history by checking the relevant boxes, including Fire, Police, EMS, and others.
3. Complete the service details by specifying the service start and end dates in Month-Year format. Include the department and position held by the nominee. If additional information is required, attach a separate sheet.
4. In Section B, select the appropriate award by marking the AESM or the relevant service bar for 22, 32, or 40 years of service.
5. Section C requires the nominator's information. Fill in the surname, first name or initials, position, mailing address, telephone number, and email. Certify the nominee's service and eligibility by signing and dating the section.
6. For internal use in Section D, if applicable, provide the surname, first name or initials, position, municipality address, and telephone number. Indicate the recommendation status as Yes or No, and date the approval section.
7. After completing the form, ensure all required documents, such as the letter of confirmation from each department listed in Section A, are attached. Save your changes and prepare the final submission.
8. To submit the nomination form, you can either email the completed form to the relevant departmental emails provided or mail the hard copy to the Office of the Fire Commissioner.
