9. Indicate if your AEP application has been previously denied or if it has been cancelled/revoked, and provide details regarding the denial or cancellation.
10. Sign and date the application form as the applicant.
11. The employer should fill in the endorsement section with the name and signature of a company officer, along with the position in the company.
12. Finally, save your changes, download, print, or share the completed form as needed.
