Triangular flap is the most common approach used by many surgeons. The disadvantage is that the triangular flap has difficulty to achieve primary wound closure on healthy bones, making it possible for dehiscence and complications such as alveolar osteitis.

A flap in technical terms is a tissue that is transferred from one side (donor site) to another (recipient site) while main- taining its own blood supply. Depending on the position of the donor and recipient sites, flaps can be broadly classified as local or distant (regional) flaps.

Currently there are three types of periodontal flaps available, Modified Widman flap, Undisplaced flap and Apically-displaced flap.

Several incision designs are commonly used to perform an apicoectomy. The first we will discuss is the semilunar flap (Figure 15-3,A). Classically, this flap is used in the region of the anterior maxilla. It is a semicircular flap with the concave portion of the incision hanging cervically.

Classically, the most commonly used type of flap in periapical surgery has been the trapezoidal or triangular Neumann flap, with an intrasulcular incision and two vertical releasing incisions.

Oral surgical flap by definition is the operation in which a portion of the mucoperiosteal tissue is surgically detached from the underlying bone for better access and visibility.
