1. Press the ‘Get Form’ button to access the form and open it in your preferred editor.
2. Begin by selecting the course title. You will find various options available; please tick only one course that you wish to register for.
3. Fill in the company details section, including the company name, email address, trade license number, and contact number.
4. In the participant details section, enter the participant name, their designation (to be shown in the training card), and indicate whether it is a new registration or a renewal. If it is a renewal, provide the old card number.
5. Specify the gender of each participant in the corresponding field.
6. Ensure to gather and prepare the required documents: Company Trade License, Passport copy with valid visa page, Passport size colored photo in JPEG format, Detailed CV (if applicable), and Authenticated Academic and Professional Qualification certificates (if applicable).
7. Remember to format all documents (except the photo) in PDF format and name them appropriately, such as ‘Application Form_Candidate Name’.
8. Once you have completed the form and attached all required documents, send the entire package to EHS.Accreditation@trk.pcfc.ae.
