The ISMP National Medication Errors Reporting Program (ISMP MERP) is an internationally recognized program for healthcare professionals to share potential or actual medication errors that occurred at their workplace. Reporting an error or hazardous condition is simple and confidential.

Drug error means an adverse drug event or a drug incident where the drug has been released to the patient. Patient information. Incident date & discovery date. Name of reporter & incident discoverer. Drug ordered. Incident description. Severity.

Any staff member who discovers a medication error, whether a physician, pharmacist or nurse, must immediately complete the Medication Error Report (Appendix I). The details include; patient name, hospital number, prescription details, details of errors and any incorrect medicine or dose administered to the patient.

This form is for use within the pharmacy to record details of medication safety incidents that relate to errors in the pharmacy (i.e. not external errors such as prescribing errors).

It can include the following: Medications given to the incorrect client. Incorrect medicine being given. Incorrect dose being given. Incorrect time of medicine. Incorrect route of medicine. Spilt or dropped medicine. Out of date medicine. Missing medicine.

This report should include: client's ID, name and dose of med, time and place of incident; accurate and objective account of the event, who you notified, what actions you took, your signature (or that of the person who completed the report). Do not reference or include this report in the client's medical record.
