Zung's Self-rating Anxiety Scale (SAS) is a norm-referenced scale which enjoys widespread use a screener for anxiety disorders.

Each question is scored on a Likert-type scale of 1–4 (based on these replies: "a little of the time", "some of the time", "good part of the time", "most of the time"). Some questions are negatively worded to avoid the problem of set response. Overall assessment is done by total score.

Validity and Reliability The SAS-ID was given to 136 adults with intellectual disabilities and to 96 caregivers. The internal consistency reliability coefficient was . 80 and the correlation rating with the Fear Survey for Adults with Mental Retardation was 0.40, .

The researchers concluded that the scale is effective in identifying severe depression. However, a 2020 study looking at patients with psoriatic arthritis concluded that the Zung self-rating depression scale might not be the most effective form of assessing depression levels.

The following guidelines are recommended for the interpretation of scores: 0–9, normal or no anxiety; 10–18, mild to moderate anxiety; 19–29, moderate to severe anxiety; and 30–63, severe anxiety.

The SAS is a 20-item self-report assessment device built to measure anxiety levels, based on scoring in 4 groups of manifestations: cognitive, autonomic, motor and central nervous system symptoms.
