High A1C levels are a sign of high blood glucose from diabetes. Diabetes can cause serious health problems, including heart disease, kidney disease, and nerve damage. But with treatment and lifestyle changes, you can control your blood glucose levels.

Total glycated hemoglobin (Total GHb) refers to all the glycated hemoglobins, including glycated hemoglobin variants. Total glycated hemoglobin is usually determined by affinity chromatography or immunassays. Hemoglobin A1c (HbA1c) is the major subfraction of the glycated normal hemoglobin (HbA1).

A new laboratory test, glycohemoglobin determination, can be used to monitor long-term blood glucose levels with a single venipuncture, regardless of whether the patient is in a fasting state.

The formula for conversion of Hb A1C to glucose in mg/dL is:Estimated average glucose (eAG) = (28.7 x A1C) – 46.7The HbA1C measured on a patient is reported as 7.5%.

Conclusions: GHb is a highly specific and convenient alternative to fasting plasma glucose for diabetes screening. A GHb value of 2 SD above the normal mean could identify a high proportion of individuals with undiagnosed diabetes who are at risk for developing diabetes complications.

An HbA1c of 48mmol/mol (6.5%) is recommended as the cut off point for diagnosing diabetes. A value of less than 48mmol/mol (6.5%) does not exclude diabetes diagnosed using glucose tests. In patients without symptoms of diabetes the laboratory venous HbA1c should be repeated.

The calculator below can be used to estimate the average blood sugar level based on the measured percentage of glycosylated hemoglobin (HbA1c). FORMULAS : Average Plasma Blood Glucose (mg/dl) = (HbA1c * 35.6) - 77.3. Average Plasma Blood Glucose (mmol/L) = (HbA1c * 1.98) - 4.29.
