The Patient Continuity of Care Questionnaire (PCCQ) is a generic questionnaire developed to cover all three dimensions of continuity of care described by Haggerty et al5 6, and it addresses both the period before and after hospital discharge.

Promoting continuity of care includes sharing thorough and concise patient medical information amongst providers and building trust between patients and their healthcare team. It also includes ensuring a patient has safe, coordinated transitions between different healthcare facilities and providers.

There are three types of continuity that are present in all healthcare settings: Informational, management and relational.

The most common method of measuring continuity is the Usual Provider of Care (UPC) index. This requires a number of appointments per patient and a relatively long timeframe for accuracy, reducing its applicability for day-to-day performance management.

Continuity of Care (COC) measures the extent to which an individual patient sees a given provider over a specified period of time. This involves the creation of a Continuity of Care Index (COCI) that identifies the number of providers providing service to a patient and the percentage of care provided by each provider.

It includes the following sections, each of which contains several data elements: Advance directives; Alerts; Encounters; Family history; Functional status; Immunizations; Medical equipment; Medications; Payers; Plan of care; Problem; Procedures; Purpose; Results; Social history; Vital signs.

Continuity of Care in Nursing Following up with patients after they are discharged. Participating in patient discharges or transfers. Recording patient information in medical records and on transfer and referral forms. Guiding patient care using tools such as care maps and care plans.
