- Once all details are filled in, email the completed form to cmedsreturns@hmebc.com if you are in the Lower Mainland or Vancouver Island. For other areas, fax the form to the nearest sub dealer as listed on the website.
- After emailing or faxing the form, ensure to save any changes made. Users may choose to download or print the form for their records.
