- Specify if the equipment is damaged by selecting 'Yes' or 'No'. If the equipment is heavily damaged, note that it will not be returned and may be disposed of by the client.
- Once all details are filled in, email the completed form to cmedsreturns@hmebc.com if you are in the Lower Mainland or Vancouver Island. For other areas, fax the form to the nearest sub dealer as listed on the website.
- After emailing or faxing the form, ensure to save any changes made. Users may choose to download or print the form for their records.
